The rate of fatal overdoses in the Connecticut River Valley exceeded the average national rate by more than 20% with 25.2 fatal overdoses per 100,000 between 2018 and 2020. This project introduced the first substance use prevention, harm reduction and access-to-treatment services for adults and families in this area, through partnerships with Connecticut State Police, Office of Probation Services, Connecticut Harm Reduction Alliance and a network of prevention councils. It provides post-overdose outreach, connection to services through home visits by an outreach coordinator/state trooper team, implementation of Handle with Care model, mobile harm reduction services and organizational infrastructure.

Learning Objectives:

Identify how to use evidence-based tools to understand social determinants and stages of change when working with overdose survivors.
Demonstrate how to merge evidence-based frameworks to create a holistic community approach to overdose response and prevention, utilizing community data sources.
Describe how overdose prevention and response fits into the Institute of Medicine’s (IOM) Continuum of Care model.
