Making meaningful and sustainable changes in clinical practice is always challenging. It can be especially challenging when working with practices that serve different populations in different health systems. The Michigan Pediatric Epilepsy project led by Michigan’s Title V agency has successfully facilitated Quality Improvement in numerous practices – pediatric and adult primary care and neurology clinics – through a collaboration with patient and family advisors, clinical teams, and statewide organizations. Project management, quality improvement, and family engagement strategies that have facilitated improvements in telehealth, health care transition, collaboration and communication between primary and subspecialty care, and shared decision making will be discussed during this workshop.
