In our study of 100 women with severe preeclampsia, 29% had hyponatremia. Mild(130-134meq/l) in 51.7% and moderate (125-129meq/l) hyponatremia in 48.3% women. Maternal complications- HELLP syndrome, placental abruption, and preterm delivery were higher in group A(hyponatremia)(34.5%, 10.3% and 79.3%) compared to group B(without hyponatremia) (19.7%,4.2% and 60.4%) respectively. Fetal growth restriction and abnormal doppler changes were observed more in group A (31% and 20.7%) than group B (22.9% and 17.9%).
Respiratory distress, neonatal hyponatremia, jaundice, feeding difficulties occurred in 41.4%, 55.2%, 41.4%, 41.4% neonates in group A and 18.6%, 1.4%, 14.3%,17.1% in group B respectively. Maternal hyponatremia was significantly associated with neonatal hyponatremia, respiratory distress, jaundice, feeding difficulties (p < 0.05). There was no statistically significant association with low birth weight, sepsis and hypoglycemia.
Conclusion: Hyponatremia was seen in 29% women of preeclampsia with severe features. These patients developed complications like HELLP syndrome, placental abruption, eclampsia and preterm delivery, FGR, pathological doppler changes and IUFD more than those with normal serum sodium values. Also significant association was observed between maternal hyponatremia and neonatal hyponatremia, neonatal jaundice, respiratory distress and feeding difficulties.
