The aim of our study was to compare obstetric and neonatal outcomes of laparoscopic surgeries performed during pregnancy according to trimester.
Study Design: This was a retrospective cohort study between April 2011 and March 2020 in a single university-affiliated medical Centre. The study included all women who underwent laparoscopic abdominal surgery during pregnancy and delivered at our hospital. Obstetric and neonatal outcomes were compared between women who underwent surgery at first, second and third trimester of pregnancy.
Results:
A total of 180 women had laparoscopic surgery during pregnancy; 95 (52.78%), 78 (43.33%) and 7 (3.88%) at first, second and third trimester respectively. Median Gestational age at surgery was 13.3 weeks (IQR 9.3-18.5). Ninety-seven patients had adnexal surgery (53.8%). Baseline characteristics and obstetric history were comparable between groups (table 1). More patients underwent adnexal surgery at first and third trimester compared to mid trimester. There was increasing trend of using Hasson entry technique at second and third trimesters compared to first trimester. Surgical site infection rate was higher at third trimester, however other intraoperative and postoperative complications were comparable. Pregnancy and neonatal outcomes were also comparable (table 2); except of lower birth weight at first and second trimesters compared to third trimester; however, rates of small for gestational age newborns were comparable between the groups.
Conclusion:
Laparoscopic abdominal surgery during all pregnancy trimesters is feasible and relatively safe. Moreover, pregnancy outcomes following surgery are comparable between the three trimesters, except of lower birth weight at first and second trimesters.
