MTS3 - Value Based Peer-led Population Health Management

Most Medicaid members with an SMI and high spend are unengaged in outpatient behavioral. Peers are uniquely able to engage these individuals, but many people are not aware of their potential. Since firsthand launched last year, the company has taken responsibility for 55,000 attributed individuals in three states in partnership with MCOs. Firsthand employs peers with firsthand experience who can connect with individuals, build trust, and introduce benefits at the right time. Firsthand uses a data-driven, boots-on-the ground approach to community engagement. A list of questions for panelists on the following topics: individual peer introductions, firsthand questions, peer perspectives, firsthand and community providers, and a bold vision for individuals with SMI. Panel Questions: What are your individual backgrounds? What is firsthand’s mission? What do you mean by SMI? What diagnoses count? In what communities is firsthand available to individuals with SMI? Which states and MCOs are taking a more innovative approach on SMI? Which National Council members are leaders in SMI care models? How does your firsthand experience serve as a superpower for your work? When and where can peers be more effective than community health workers or other providers? How does firsthand accommodate employees living with SMIs and any mental disabilities? Why would National Council member providers want to collaborate with firsthand? How can organizations be more peer-led? Is firsthand growing to become the largest employer of peers in the country? If 10 years from now every MCO covered firsthand services in every state, how would life be better for individuals with SMI? Target audiences: CMHCs/FQHCs, MCOs, employers of peers and other community resources.
