While the terms "integration" and "whole health" have proliferated, actual integrated and whole health care has not. Integrated, whole health services are the end result of an intentional and ongoing effort that requires re-working organizational culture, infrastructure, training and service delivery. Who knew it would be so hard?! Cultures, paradigms, education, language and more! We will define the essential components of whole health, integrated care, key milestones along the journey, lessons learned and pitfalls to avoid for those seeking the same goal: transformed patient care that makes health and wellbeing a reality for all.

Define the elements of an integrated, whole health model and describe their distinctiveness from co-located services.
Identify the priority areas of organizational transformation needed to achieve effective integrated care.
Incorporate new concepts and lessons learned into action steps for starting or continuing your organization's progress toward whole health integration.
