By becoming familiar with the “footprint” of patients struggling with social determinants of health (SDOH), clinicians can be aware so they can screen for them when universal screening is impossible or impractical. We will present systematic data on Medicaid patients who have been screened for SDOH and present claims data on their total amount of utilization, as well as patterns of utilization, including where the service was delivered for both physical and mental health. Learn the innovative way screening was conducted, the obstacles and how data can be used to improve outcomes.
