Transition to Adult Health Care: Hawaiʻi Partnerships to Support Youth in their Transition Journey
Health care transition is important to ensure greater and equitable access to health care. Dynamic partnerships increase awareness and support for youth’s transition journey. We will share our experiences in building partnerships using Hawaiian values of ʻohana (family), kuleana (responsibility), and laulima (working together). You will learn about the many partnerships that support and engage youth and families in our communities.
Participants will:
1. Learn about Hawaiʻi health care transition efforts through partnerships.
2. Learn how to identify and develop potential partnerships in their own community.
3. Receive health care transition resources to customize for use in their own community.
