This workshop will highlight ways to promote maternal health, improve birth outcomes, and reduce the adverse consequences associated with comorbidities by engaging families to access wrap around services. Learning objectives are to: Increase understanding of the importance of comprehensive care coordination and access to support services for pregnant and postpartum women; Discuss findings that support the impact of wrap around services in alleviating barriers for birthing persons and barriers in postpartum healthcare services; Review lessons learned from the Howard University team on developing a hospital-based perinatal care home-visiting program; Provide a foundational framework for other cities and organizations to create wrap around services for implementation in their area.
