Watch and wait has gained popularity especially for patients with low rectal cancers that require abdominoperineal resection or coloanal anastomosis. However many questions remain including the optimal neoadjuvant regimen, timing of evaluation of response, and long-term surveillance schedules. The surgical oncologist needs to be familiar the different neoadjuvant regimens to treat rectal cancer when considering watch and wait, how to deal with near complete responses and have long-term surveillance strategies.
