Replicating Project ECHO: An Approach To Help Integrate Diabetes Self-Management in Medical Rehabilitation Programs

Many inpatients attending medical rehabilitation have diabetes as a secondary diagnosis when they attend rehabilitation for an acute problem while their hospital stay may be used to optimize their lifestyle and better diabetes care. The presenters will describe the integration of diabetes care and self-management in the inpatient experience. Project ECHO (Extension for Community Healthcare Outcomes) is a well-established virtual workforce development platform including de-identified case discussion. With an ECHO session, attendees can learn from a short didactic presentation and through the discussion with clinical experts; they can also contribute to the brainstorming processes. The short didactics can be used to disseminate and integrate diabetes self-management curricula, such as the “ADCES7” - diabetes education framework in medical rehabilitation. It can be used to build capacity and expertise around diabetes self-management in the inpatient rehabilitation workflow with the goal of reducing the risk of further diabetic complications.
