The Improving Medicare Post-Acute Care Transformation (IMPACT) Act of 2014 mandated the modification of post-acute care (PAC) patient assessment instruments, including the Inpatient Rehabilitation Facility Patient Assessment Instrument (IRF-PAI), to enable the reporting of standardized data with regard to quality measures and specific clinical categories across all PAC providers. In addition to IRFs, the PAC settings addressed by the IMPACT Act include Long-Term Care Hospitals (LTCHs), Skilled Nursing Facilities (SNFs), and Home Health Agencies (HHAs). This symposium will present research conducted in all four PAC settings on behalf of the Centers for Medicare & Medicaid services (CMS) to identify, develop, refine and test candidate cross-setting assessment items specified in IMPACT Act. We will provide an overview of the project and will present national testing results for assessment of: 1) pain, 2) cognitive function, 3) depression symptoms, and 4) medication reconciliation. Implications for implementation across PAC settings will also be discussed.
