Abstract: Quantitative and objective determination of dental pain is an unmet clinical need. We previously reported that skin conductance, known as electrodermal activity (EDA), has the potential to measure sensations triggered by pulp vitality tests and the reduction of toothache after successful local anesthesia. EDA can be recorded by laboratory-scale devices like PowerLabTM combined with a galvanic skin response (GSR) amplifier. The popularity of EDA for detection of dental pain may be impacted by the size, usability and cost of this device. Here, we determined whether a wearable EDA device (Shimmer3TM) is in reasonable agreement with the PowerLab for pulpal diagnosis. We recruited 20 patients (normal pulp n=43, pulpitis n=10, pulp necrosis n=11) following inclusion and exclusion criteria. We recorded EDA simultaneously by PowerLab and Shimmer3 in conjunction with cold test and EPT. The raw data was processed and computed the modified time-varying index of sympathetic activity (MTVSymp), a highly sensitive EDA index for pain. The degree of agreement between Shimmer3 and PowerLab and clinical diagnosis by cold test and EPT were analyzed using Cohen’s Kappa analysis. Our results showed an overall moderate to good agreement between the Shimmer3 and PowerLab in cold (k=0.5) and EPT tests (k=0.65). EDA recorded by either system identified 85-92% of necrotic pulp. MTVSymp recorded by PowerLab showed better agreement with the results of cold test (k=0.75) than Shimmer3 (k=0.5). Further studies will focus on enhancing the specificity of EDA for dental pain and improving the performance of a mobile EDA device.
