At the end of this session participants will be able to / Au terme de cette conférence, vous serez:

identify key members of the multidisciplinary team and describe their role in the management of ACS;
evaluate strategies used to create a successful treatment plan post-ACS; and
identify key barriers to treatment success and integrate strategies for optimal adherence post-ACS.

Workshop Description: Each presenter will review their role in the management of Post-ACS care.
Strategies used to ensure each patient's treatment plan has been addressed.
Barriers discussed by each role and how they address to ensure optimal success in adherence upon discharge from hospital.

Sharing and reflection by participants to incorporate strategies they could utilize or have found to work in their facility.
(15 minutes)
