At the end of this session participants will be able to / Au terme de cette conférence, vous serez:

To illustrate the perspectives of individuals who deal with complex psychosocial challenges and why they may not be able to engage in their self-management.
To describe an empirically derived novel conceptual framework for delivering diabetes care to people who face social disadvantages - focusing on relationship-based and trauma-informed care.
To contrast the results of the ACCESS trial which showed that a remote self-management education and support program created by an advertising agency had a greater impact on health outcomes in older adults living with low-income than the provision of free medications.
To appraise our practice of motivational interviewing and behaviour change techniques to support individuals that face many challenges.
