identify key evidence-practice gaps in secondary prevention and follow-up care for patients with acute coronary syndrome;
understand why implementing evidence-based secondary prevention in the real-world is challenging; and
understand how to address evidence-practice gaps in secondary prevention at the patient-, healthcare provider-, and health system-level.

Session Description: Within our health system, there are considerable gaps in the implementation of existing, evidence-based secondary prevention programs for patients with acute coronary syndrome (ACS). As a result, many patients fall through the cracks upon leaving hospital and either do not or cannot access the necessary support and services needed to improve their cardiovascular health. We need to know more about what prevents the implementation of evidence-based secondary prevention in the real-world and develop solutions to improve access and adoption among patients, healthcare providers, and health systems.

As part of a 2022 CIHR Planning and Dissemination Grant, we held an expert meeting with Canadian and international colleagues to discuss the state of the science, develop a multidisciplinary research agenda in this topic area, and build on existing and forge new collaborations; to ultimately improve the care of patients with ACS. We see our proposed CCS workshop as an opportunity to further engage with key stakeholders with valuable real-world experience to help inform this innovative program of research moving forward.

During this interactive workshop, participants will hear from a range of experts in cardiology (JD Schwalm), cardiac rehabilitation (Neville Suskin), medicine (Tim O’Shea), pharmacy (Claudia Bucci), nursing (Diana Sherifali), and health psychology (Alison Phillips). One example presentation will focus on innovative ways healthcare providers can help patients build self-management/prevention habits in their home environments, for optimal adherence and long-term maintenance. Our aim for this workshop is to share key findings from our collaborative work to date, learn from key stakeholders about their real-world experiences, and generate meaningful discussions on how to move the science forward in the field.
