At the end of this session participants will be able to / Au terme de cette conférence, vous serez:

identify the challenges to effective transition of care from pediatric cardiology to adult cardiology;
be aware of the recent research undertaken in the transition of care from pediatric cardiology to adult cardiology; and
have a framework for the appropriate transition of care from pediatric to adult cardiology.

Workshop Description: The transition of care from adolescence to adulthood routinely occurs within congenital cardiology but gaps remain in care. This plenary workshop will survey the current landscape as it pertains to congenital cardiology but will also address issues arising in the transition of care in other cardiology patient populations such as cardiomyopathies.

The process of transition involves not just the transfer of care to the adult clinic but also involves improving the individuals knowledge about their heart disease as well as developing an individual’s self-management and self- advocacy skills and giving them a clear pathway in navigating the complexities of the heat care system.

This session will explore these issues and the latest research on the subject.
