Pregnancy associated mortality continues to rise in the USA. Recent data from 2018-2020 illustrates a rise to 23.8 per 100,000 live births and significant health disparity with non-Hispanic Black carrying the greatest burden of disease (National Center for Health Statistics). Although causation has not been reported in relation to this recent data, cardiovascular disease (CVD) related maternal mortality has been recognized as the number one cause of indirect maternal death in the USA (Obstetrics & Gynecology 2015;125:5-12). California Pregnancy Related Mortality Reviews indicates that delays in health care response contributed to 61% of these maternal deaths and a significant number of these deaths could have been prevented with timely recognition and treatment (ACOG 2015;213:379e1-10). There is a need for Maternal Fetal Medicine Fellows in training to understand pathophysiology of CVD, recognition and triage of pregnant/postpartum patient with signs and symptoms suspicious for CVD, and referral to the appropriate level of maternal care.
The focus will be on short, rapid fire expert lectures highlighting clinical pearls useful in day-to-day practice setting. Will review the top 5 learning points for understanding cardiovascular pathophysiology, top 5 most useful cardiac diagnostic tests and their interpretation in pregnancy. Cardiac case presentations guided by the decision-making tools by the Experts and Fellows will create an interactive session to strengthen clinical concepts using multidisciplinary approach at all levels of care. The goal is to maintain a dynamic learning environment and review the whole spectrum of cardiac disease in pregnancy including clinical implications of physiologic changes of pregnancy, preconception/early pregnancy cardiovascular risk assessment, diagnostic testing, specific cardiac lesions and creation of a Cardio-Obstetrics team.
