Between Sept 2020 and Dec 2021, a total of 1323 patients were enrolled and completed the GAD-7 screen with 163 in the PTB and 1160 in the WHC cohorts. There were significantly higher GAD-7 scores in the PTB cohort compared to the WHC cohort (median score 4 [1,9] vs 2 [0,5], p < 0.001). The significant association of GAD-7 persisted when examining across both prior PTB and in those delivering < 35 weeks in the current gestation (Figure). There was a higher rate of positive screens in the PTB group compared to WHC (20% versus 13%, p = 0.01, Table).
Conclusion:
At entry to prenatal care, patients with a history of prior spontaneous PTB have significantly higher anxiety symptoms compared to low-risk controls. Those with recurrent PTB < 35 weeks in the current pregnancy had the highest anxiety screening scores.
