(581) Prenatal maternal hyperemesis gravidarum and long-term neurological morbidity of the offspring: a population-based cohort study

Objective: Hyperemesis gravidarum (HG) is one of the leading causes for hospitalization and morbidity during pregnancy. Intrauterine environment has an important role in shaping fetal development, still not much is known about the effect of maternal HG on long-term offspring morbidity, and specifically neurological morbidity. The study objective was to clarify whether offspring exposed to maternal HG are at an increased risk for neurological -related hospitalization .

Study Design: A population-based cohort analysis was performed comparing neurological morbidity of offspring of mothers with and without HG. Data were collected from the perinatal and pediatric databases of a single tertiary medical center. All singleton infants born between the years 1991-2022, who belong to the largest HMO in the country, were included in the study. A Kaplan-Meier survival curve was used to compare the cumulative incidence of neurological morbidity, and a Cox proportional hazards model was constructed to adjust for confounders

Results: The study population included 232476 pregnancies, 3225 (1.4%) were complicated with HG. The incidence of total neurological morbidities was higher among the HG as compared to the non-HG group (10.4% vs. 7.8%, OR=1.36; 95%CI 1.22-1.53, p< 0.001, Kaplan-Meier log rank, p< 0.001; Figure). Specifically, eating and emotional disorders were more common in the HG offspring (Table). In the multivariable Cox model, offspring born following pregnancies complicated with HG were more likely to suffer from neurological morbidities (adjusted HR=2.18; 1.98-2.41, p< 0.001controlling for gestational age, maternal age, cesarean delivery and ethnicity.

Conclusion:

In this large population based study, offspring exposed to maternal HG are at an increased risk for neurological morbidities. These findings suggest that early life exposures may have long-term effect on the neurological development of the offspring, andmaight emphasize the importance of proper treatments for HG.
