(602) The Impact of Continuous versus Traditional Glucose Monitoring on Prenatal Care for Pregnant Diabetic Patients

Objective: The rise of continuous glucose monitors (CGMs) has led to multiple questions about its utility in the management of patients with diabetes in pregnancy. The objective of this study was to assess compliance of blood glucose monitoring during prenatal care by comparing traditional glucose monitoring (TGM) to CGMs in pregnant pregestational diabetic patients. Our hypothesis is that patients that used CGMs are more likely to record and report their blood sugars than those that used TGM.

Study Design: A case-control study was performed from 2020-2022 of pregnant patients with Type 1 or Type 2 pregestational diabetes at an outpatient practice affiliated with an urban tertiary care hospital. The primary outcome was the percentage of visits that the glucose log was reviewed. The percentage of blood sugars reviewed was defined as the number of visits at which blood sugars were reviewed over the total number of prenatal visits attended. Additional information was collected about maternal medical history, prenatal vitals, laboratory values, and diabetes management medications. All individuals with a stillbirth prior to 28 weeks were excluded. Data were analyzed according to distribution with ANOVA for continuous variables and Fisher’s exact tests for categorical variables.

Results: A total of 89 individuals met final inclusion criteria, 59 (65.2%) used TGM and 30 (34.8%) used CGMs. The percentage of logs reviewed improved by 29% in the CGM group (38.8 % with TGM and 68.2% with CGM, p< 0.001). There was a significant increase in the blood sugars reviewed at greater than 50% of appointments (33.3% in TGM and 73.3% in CGM; OR 5.4, 95% CI: 2.1-14.2). The rate of postpartum visit attendance was also higher in individuals using a CGM (72% in TGM and 93% of CGM, p < 0.001) (Table 1).

Conclusion: Blood glucose log review at prenatal visits was significantly higher in pregestational diabetic patients who used CGMs compared to TGM, indicating that CGMs may improve compliance outcomes amongst pregnant patients with pregestational diabetes.
