Total of 18,999,808 live births including 2,430 isolated MMC were investigated during the study period. Isolated MMC prevalence increased as BMI increased with highest being among class III obesity (1.88 per 10,000 livebirths). Following the three levels adjustment, pre-pregnancy overweight and class I, II, III obesity remained significantly associated with isolated MMC. The adjusted odds ratios were: 1.32 (95% CI 1.19-1.46; p< 0.001) for overweight, 1.55 (95% CI 1.38-1.75; p< 0.001) for class I obesity, 1.68 (95% CI 1.45-1.94; p< 0.001) for class II obesity, and 1.73 (95% CI 1.47-2.04; p< 0.001) for class III obesity. Test of trend showed significant increase in prevalence of isolated MMC associated with class II and III obesity over the past 5 years.
Conclusion:
We have demonstrated significant increasing trend of fetal isolated meningomyelocele in pregnancies with overweight and class I/II/III obesity over the past 5 years after adjusting for other covariates including socioeconomic, demographics, and metabolic characteristics. Future studies are needed to seek etiological explanations and to develop improved methods for primary prevention.
