Aortic dissection with malperfusion presents acutely ill patients with life-threatening aortic and peripheral arterial injuries. This session will compare anatomical mechanisms of arterial obstruction in acute aortic dissection, treatment priorities and goals, treatment tools strategies, and post-procedural goals. The goal is to foster a close working relationship between the emergency room, cardiothoracic surgery, interventional radiology, and vascular and general surgery in the treatment of these patients.

Learning Objectives:

review a CT and recognize ischemic anatomy in the relationship of the dissection flap with aortic branch arteries, and formulate a preliminary plan to revascularize the abdominal and pelvic arteries
the roles of fenestration and aortic true lumen stenting in the treatment of dynamic obstruction, and the roles of stenting and thrombolysis/thrombectomy in the treatment of branch artery static obstruction, and appreciate roles of selective arteriography, intravascular ultrasound of aorta and branch arteries, and manometry of aortic root and branch arteries.
distinguish between immediate goals of vascular intervention for dissection versus vascular interventions for obstruction due to embolism or trauma
