Development and refinement of the CCBHC model would benefit from formal data analysis that includes program evaluation from external partners with expertise in public health evaluation. Realizing the vision of health equity requires academic and community partnerships that foster multidirectional learning and mutual innovation beyond single disciplines, cultural norms, political will and sectoral and geographic boundaries. CCBHC grantees will articulate the importance of data analysis for organizational decision-making, summarize the key aspects of the person-centered CCBHC model, illustrate critical components of the community-academic cross-sector partnership and illustrate how evaluation and monitoring are linked to program performance.
