TP36 - Integrated Care in a Community-based Organization: Certified Community Behavioral Health Clinic in Practice

Community-based organizations (CBOs) often feel overwhelmed when making the shift to integrated care. The belief that it is an “all or nothing approach” hinders them from moving forward on the spectrum of integrated care. Volunteers of America (VOA) is a large community-based organization providing health and human services across local sites in the United States and Puerto Rico that creates programming to respond to the unique needs of the communities we serve. Find out how two local VOA sites implemented integrated care through the CCBHC mode. You will take part in a group exercise to identify where you are on the continuum of integrated care, identify other examples of integrated care outside of CCBHCs, take away at least one idea of how to move toward integrated care in their work and share two lessons learned from VOA sites’ implementation of CCBHCs and integrated care.
