Daymark recently became a Certified North Carolina Care Management Agency. Care Managers in this program are responsible for the Whole Person, and as such, we as an agency are responsible for the Health Quality Measures for every Medicaid Member assigned to us. Medicaid members are assigned to a "best fit" agency, whether the member is a patient of that agency or not. This poses many challenges: 1) How do we discover health care data in a real-time fashion? 2) How do we become aware of Inpatient and Emergency Department admissions in real-time from 13 hospital systems in the state? 3) How do we take a workforce across 30 North Carolina counties, and make them immediately aware of circumstances happening across thousands of points of care in an organized and actionable manner?
