Post-traumatic stress disorder (PTSD) has a profoundly negative impact on mental health, quality of life and day-to-day functioning. Providers need evidence-based practices (EBP) that can be implemented and sustained through their community-based programs that serve vulnerable populations most in need of these interventions. We will a successful partnership between a funder, a training organization and an independent practice network and the successful training and implementation of CPT across a network of care.

Learning Objectives:

Describe implementation process and strategies to develop successful partnerships and implement evidence-based practices within care networks.
Identify best practices to achieve outcomes and key outcomes relevant to value-based payments and how to develop tools to track outcomes.
Apply lessons learned and best practices to improve upon or adopt a new implementation process for an EBP.
