In response to the emergency department boarding crisis, New Hampshire's Department of Health and Human Services (NH DHHS) implemented Critical Time Intervention (CTI), a case coordination intervention bridging the “critical time” from psychiatric inpatient to community reintegration. CTI helps people during transitions build trusting relationships within a comprehensive support network, developing and strengthening connections to long-term resources and leading to happy and productive lives in the community. The presentation will cover the CTI model and history in NH, implementation strategies, including missteps and learning experiences, and emerging data from the statewide initiative.
