The presentation examines the unique aspects of the design and delivery of care coordination for children, young adults, and their families. Historically, care coordination efforts for families have focused primarily on youth with complex behavioral needs and their families through intensive care coordination using Wraparound. Modernization of care coordination approaches need to include a tiered care coordination approach which involves working with a small number of children, young adults, and caregivers with moderate and complex needs often spanning multiple social determinants of health, possible multi-system involvement, and potentially at-risk of out-of-home care.
