This session will use collaborative, co-learning methods to explore the value of and rationale for centering the community in the development and use of evidence to guide Title V decision-making. A broad definition of evidence will be explored and supported by using the Medical Home Community Team program as a case study. Participants will have the chance to hear about and share their own experiences related to evidence generation and use. Relevant resources, supports and tools will be shared by the presenters.
