Indiana University Department of Pediatrics has developed the Indiana Complex Care Coordination Collaborative coaching curriculum to embed nurse care coordinators in primary care to serve children with medical complexity, initially in 3 sites with planned expansion into 24 pediatric and adult practices. The curriculum includes orientation, first six-month training, and ongoing learning collaborative. A didactic series, case discussions and point-of-care resource playbooks are the key components initially. Quality improvement and shared skill expansion continue in the collaborative. IUSM team members and state partners will provide a panel presentation of model creation and evolution, including lessons learned and key partnership contributions from Indiana Family to Family and Title V.
