Come learn about the Medical Home Portal (medicalhomeportal.org) and gain a better understanding of what it is and how it can be used to support professionals and families in working together to care and advocate for CYSHCN. Successes from a state partnership with Utah will be detailed and highlighted during this workshop. Participants will learn how the Medical Home Portal can support state Title V MCHB Block Grant goals and see how successful collaborations with other family-focused organizations can help stretch resources, reduce duplication of efforts and make it easier for families to navigate information and find services, supports and specialists.
