A Colorado CYSHCN Construction Project: Building Tools that Drive Meaningful Change & Cultivate Diverse Leaders in MCH

Providing MCH-funded care coordination for CYSHCN has been a longstanding function of local public health agencies in Colorado. As the health system landscape changed, some agencies re-examined their role in community-based systems of support for CYSHCN. This session highlights the Colorado CYSHCN Decision-Making tool designed to guide community conversations that identify population needs and facilitate conversations about who in the community can best meet those needs. Colorado will share the process for establishing the workgroup that developed the tool - including strategies to promote language justice and inclusive engagement practices for people with lived experience - as well as share the Community of Practice that was developed to promote shared learning at the state and community level.
