This virtual poster will present on the academic-practice partnership (APP) that was established in 2014 by the University at Albany Maternal and Child Health Program and the New York State Title V program at the Department of Health. The poster will cover the origins of the partnership, partnership structure, current activities, challenges, and lessons learned. The presentation will conclude with specific recommendations for other academic institutions and Title V programs who would like to create a similar partnership.
