Soft tissue changes in hyperdivergent patients are largely dependent on retraction of anterior teeth and vertical control, in other words, intrusion of the teeth. The critical point for facial profile changes is related to disappearance of hypermentalis muscle, which is normally evident on skeletal Class II hyperdivergent patients. The retraction of the anterior teeth and vertical control can reduce the length of distance in hard tissue from basal maxilla through incisal tip of the upper incisors, to the chin, while the soft tissue length is not changed, and hypermentalis muscle activity can be disappeared. There are many factors affecting the face changes in this treatment.
The long-term stability of hyperdivergent treatment was always questioned because of high possibility of relapse. Speaker would like to explain how to maximize the profile improvement in hyperdivergent patients and to emphasize the importance of soft tissue dimension on facial esthetic improvement and its long-term stability.
