OR27 - Pulpal Diagnosis and Prevalence of Maxillary Sinus-Related Abnormality in Patients Receiving Endodontic Treatment

Evaluate association between pulpal conditions of maxillary molars and maxillary sinus mucositis among patients who received endodontic care at UMKC Endodontics Residency Clinic.

Methods: After IRB approval, CBCT images and axiUm charts of endodontic treatments between January 2014 and December 2021 were reviewed. Clinical features such as pulpal diagnosis, presence of missed canal, distance from the maxillary sinus floor and root tip, distance from the maxillary sinus floor and periapical radiolucency (PARL) and presence of mucositis (defined as maxillary sinus mucosal thickening) were determined from axiUm chart and CBCT scan. A chi-square and logistic regression analysis were performed to determine if mucosal thickening was associated with pulp status, and its magnitude if present.

Results: Of the 128 maxillary molar root images analyzed, 60% with a missed canal, 89% with PARL into the maxillary sinus, 44% with necrosed pulp, and 0% with vital pulp were associated with maxillary sinus thickening. Overall, all root images with missed canal (7.8%) had a PARL, but only 13% with vital pulp had a PARL. Pulp status (vital vs necrosis) was positively associated with mucositis (Fisher’s exact test p = 0.03) and the odds of presence of mucositis was 5.2 higher (95% CI = 1.3-20.3) when previous endodontic treatment had a missed canal.

Conclusions: Based on the retrospective evaluation, the prevalence of maxillary sinus mucositis was positively associated with the pulp status (pulp necrosis vs vital pulp) and status of previous endodontic treatment (missed canal vs. no missed canal).
