Introduction: The presence of a second mesial buccal canal is an important morphologic aspect of maxillary first molars. The purpose of this study is to determine the incidence of second mesial buccal canal in maxillary first molars found by endodontic residents and the difference between the first six months of the program and the last six months of the program.

Methods: Electronic dental records of 247 patients who received root canal treatment on maxillary first molars were reviewed for the detection of a second mesial buccal canal. The records reviewed were limited to the completed root canal therapy of maxillary first molars between July 2016 and June 2020. Exclusion criteria included the cases were a cone beam computed tomography (CBCT) was taken, nonsurgical retreatments, and the records that were unavailable for various reasons. A chi squared of independence was performed.

Results: 217 electronic dental records met the criteria. The overall incidence of second mesial buccal canal in maxillary first molars was 62.2% (135 out of 217). The incidence during the first six months was 58.1% (57 out of 98) and the incidence during the last six months was 65.5% (78 out of 119). This finding was found to be not statically significant (P > .05).

Conclusions: Endodontic residents’ ability to locate the second mesial buccal canal in maxillary first molars increases at the end of the residency when compared to the beginning of the residency. This finding is not statistically significant.
