Sodium hypochlorite (NaOCl) has been proven to be an effective endodontic irrigant due to its antimicrobial and tissue dissolution ability. The GentleWave® device is a high-tech delivery system used to distribute fluids, including 3% NaOCl, throughout the root canal system to thoroughly disinfect it while preserving tooth structure. The aim of this study was to compare the tissue dissolution efficacy of different concentrations of NaOCl using either the GentleWave® procedure or passive ultrasonics using a novel in vitro model. 27 standardized samples of bovine palatal tissue (average 74.5 mg and 4 mm x 4 mm x 2 mm) were placed in a customized apparatus to evaluate tissue dissolution for the GentleWave® and ultrasonic groups at concentrations of 3% and 0.5% sodium hypochlorite. The results were analyzed with t-test and ANOVA (p < 0.05). The GentleWave® group (n=7) using 3% NaOCl had all samples completely dissolved at an average time of 1 min 21 sec while the GentleWave® group using 0.5% (n=10) NaOCl had all samples completely dissolved at average time of 3 min 47 sec. The ultrasonic group (n=10) was run for 2 min and then the weight of the remaining sample was calculated and on average, 45.6% of the tissue remained. In conclusion, lowering the concentration of NaOCl from 3% to 0.5% while using the GentleWave® technology produces a clinically similar tissue dissolution rate that is still more effective than utilizing ultrasonics alone.
