The purpose of this in vitro study was to compare the mineral to collagen ratio and collagen cross-linking and observe qualitative changes within the canal lumen in three groups of extracted premolars. 28 extracted single rooted premolars were randomized to three groups: 1) negative controls 2) GW irrigation and 3) needle irrigation. The GW group was irrigated using 3% NaOCl + water + 8% EDTA + water according to the manufacturer’s instructions. The needle irrigation group was irrigated with 3 ml 5.0% NaOCl, 3 ml 17% EDTA, followed by 3 ml of 5.0% NaOCl. The dentin composition was analyzed using Raman spectroscopy. Sectioned halves of three GW irrigation samples, three needle irrigation samples, and two additional negative control samples were set aside for SEM evaluation. No significant differences in the average mineral-to-collagen ratio and average collagen cross-linking ratio were noted between the control, GW, and needle irrigation groups in each third of the root. SEM imaging showed that the GW irrigation samples had less debris and less intertubular dentin compared to the needle irrigation samples. Our Raman analysis showed that GentleWave™ irrigation does not change chemical properties of dentin which are associated with decreased fracture resistance compared to needle irrigation. This suggests that GentleWave™ irrigation may not increase risk of fractures in root canal treated teeth. However, further studies are necessary to determine if GentleWave™ irrigation has an impact on the mechanical properties of teeth.
