F-9 - Direction of Endodontic Instrumentation and Enhanced Irrigation

The preservation of tooth structure and the eradication of microbial biofilm continue to be the most pressing issues in endodontics. Research continues to have its primary focus on addressing these two issues. These requirements for successful endodontics are not mutually exclusive and should be thought to exist in harmony together. Mechanical instrumentation remains paramount for disruption of the biofilm as well as creation of a fluid pathway for irrigation. File geometry plays a significant role in the performance of the instrument. The geometry also plays a role in the amount of inorganic debris created which can complicate bacterial removal. Flow rate, shear stress, microbial activity and safety are all important for irrigation. Evaluating the strengths and weaknesses of instrumentation and irrigation and applying the strengths of each will have a positive direction on the future of endodontics.
