Apical periodontitis is caused by bacterial infection of the root canal system. The diversity of the endodontic microbiome has been refined and redefined by new mlecular technology, and the bacterial multispecies community has been regarded as the unit of pathogenicity. In infected canals, bacteria are usually organized in biofilm communities attached to the canal walls and also extending to anatomic irregularities, in which they may be difficult to be reached and eliminated. Therefore, disinfection strategy to eliminate root canal biofilm is the primary focus in root canal treatment. In spite of some remarkable technological advances in the past two decades, root canal systems still remain to be a challenging niche to eliminate bacterial biofilm. This lecture will review the morphologic and taxonomic features of endodontic infections and discuss the current status of root canal disinfection strategies. Advanced disinfection strategies such as photo-dynamically activated microbubbles, as well as the application of non-conventional (immune modulatory) antimicrobials for enhanced disinfection efficacy in minimally prepared canals will be reviewed.
