Abstract:
Objective: This poster presentation will describe the iterative process of developing a novel Chart Review tool to assess the minimum standard of care provided by unlicensed dental therapy trainees participating in an approved Dental Pilot Project (DPP). The DPP Program allows organizations to test new or expanded roles for oral healthcare professionals before changes in licensing laws are made. The DPP program is overseen by the Oregon Health Authority (OHA). OHA is responsible for monitoring projects which include Chart Reviews of patient records to monitor for patient safety and the applicable standard of care.
Methods: The Chart Review tool was developed through an iterative process using subject matter expert input, adapted scoring criteria utilized by recognized dental examination boards for individual procedure types, and published processes for identifying adverse events based on chart documentation. Reviews are conducted on a representative sample of completed irreversible procedures using redacted charts that include pre-operative, prep and post-operative images. Calibration and debriefing meetings are held with reviewers to ensure alignment among reviewers on all measures of procedures success. Chart Reviewers include dentists from the Advisory Committee, OHSU-School of Dentistry and the Oregon Board of Dentistry.
Results: The final Chart Review tool is currently used to assess irreversible procedures completed by trainees on an annual basis for each project overseen by the DPP Program.
Conclusions: The Chart Review tool methodology allows OHA to monitor for patient safety and to determine if trainees are meeting the minimum standard of care as required by State law.
