Description: Transitioning to a value-based care approach in oral health (OHVBC) provides an accessible, equitable, and integrated path to transform the traditional fee-for-service approach that incentivizes high cost and invasive procedures to one that incentivizes high quality and preventive oral health care. Vulnerable, underserved, and underinsured communities are more susceptible to a disproportionate burden of oral diseases and would benefit from value-based care delivery that is cost-effective, comprehensive, and integrated while maintaining health and preventing disease. An online assessment tool was created to understand OHVBC readiness and knowledge to recognize support needed to initiate the transition. The readiness assessment tool was implemented with a North Carolina (NC) cohort representing 14 dental clinics providing care in 47 counties, and a Michigan (MI) cohort representing three health centers serving seven counties, to identify dental the clinics’ capacity to transform care delivery. The roundtable will discuss how the results of the readiness assessment are a catalyst for creating learning and quality improvement opportunities to identify and implement change ideas the participants chose to test to transition towards an OHVBC model.
