Description: Dentistry has historically focused on procedures with low regard to quality such as patient outcomes and oral health quality of life. In 2021, Advantage Dental+, a multi-state practice organization with 70+ practices, in five states, working under differing payment and regulatory environments utilized a change management approach and improvement science principles with the intent to drive lasting systemic organization change. Preventistry, the Advantage Dental+ model of care, systematically captures patient data for the three diseases of the oral cavity (caries, periodontal disease, and oral cancer). The presentation will explain Advantage Dental+ unique background, involvement in a dental accountable care organization, and different payment models that influenced the Preventistry framework. An overview of their person-centered, evidence-based disease management care model, and information on how they operationalized a care model to support value-based care in practices in a data-driven way. Case examples will be shared to highlight how the framework is applied.
