My name is Cathy Chacon and I am privileged to work as a registered nurse at National Jewish Health’s Adult Cystic Fibrosis Center for the past 20 years as an Adult Cystic Fibrosis Program Coordinator. In my role at National Jewish, I am responsible for coordination and facilitation of most aspects of CF care including medical, pharmacy, home health and access to care for more than 500 adult CF patients. I also participate in rounds on the inpatient service to ensure smooth transition from inpatient to outpatient care as well as see outpatient CF patient in a clinic setting three days a week. As a senior staff member and leader, I help guide and mentor new team members. I also participate in our current program with our pediatric center regarding transition to adult care and have worked closely in the past with the Cystic Fibrosis Foundation on the CF Peer Connect pilot mentorship program and the Project for Sustaining Daily Care Adherence Initiative.
