URMC Complex Care Center
Stacey L. Smith, LMSW. I completed my MSW at the University at Buffalo in 2018. I sat and passed the Social Workers' licensing exam in Feb of 2022. I have a dual bachelor's from the College at Brockport in Psychology and Health Science, with a concentration in Drug and Alcohol counseling. I have worked in the field of developmental disability from 2017 -o 2021 and am still working with these individuals indirectly at the clinic as the clinic has a large population of patients that also receive service from the office of people with disabilities OPWDD. I stated that the complex care center as an embedded care manager employed by Prime care coordination to provide services coordination for individuals with disabilities. I was embedded in the complex care center during a pilot program to allow for providers and Care managers to have closer and better communication. I transition into the role of Social Worker at the clinic in April of 2021.
I am the Social worker at complex care. I have dual duties at the URMC CF program. I am also the Mental Health Coordinator at the clinic. All CF patients should have an assessment each year to make sure they have the supports they need (access to food, housing, etc.) as well as offering information on assistance programs for medication and scholarships, to name a few of the objectives. There is also an annual Behavior Health assessment completed to assure people are not struggling with issues including depression, anxiety, and/or coping with chronic illness. I talk with patients and families to determine their needs and help connect them with needed resources.
Another very important part of Stacey’s job is the creation of a CF Program Patient/Family Advisory Council (PFAC) The PFAC is something highly desired by the CF Foundation as a way to give voice to issues patients and families may have in regard to their healthcare or specifically the clinic. I organized the PFAC and held the first meeting! The council consists of patients, family members, members of the clinic, and Highland Hospital patient advocates.
