Alanna Breckenridge PharmD, BCGP is a Transition of Care Clinical Pharmacist at Northside Hospital Forsyth in Cumming, Georgia. She works with an interdisciplinary team devoted to the care of people living with Chronic Obstructive Pulmonary Disease (COPD) or pneumonia. The Transition of Care team hosts an annual COPD Symposium and Breathe Better Events to bring more community awareness to lung diseases. Dr. Breckenridge has developed a Proper Inhaler Technique brochure and poster used to educate patients, caregivers and healthcare professionals. In her clinical role, Dr. Breckenridge engages patients, hospital staff and post-acute care partners to obtain the best possible medication history. She educates patients about their medication regimens and coordinates care to minimize high-risk medications and reduce treatment burden to optimize care. She has spearheaded numerous initiatives to improve the care and safety of patients. Dr. Breckenridge earned her Doctor of Pharmacy degree from Nova Southeastern University in Fort Lauderdale, Florida in 2000. She holds a Medication Therapy Management Certificate from University of Florida/American Pharmacist Association. She serves as a preceptor for Transitions of Care APPE pharmacy students at both the University of Georgia and Mercer University. Dr. Breckenridge was the recipient of the Josie King Foundation Safety Hero Award in 2019.
