Dr. Donna Leang, PharmD, MHDS, BCPS, FCSHP is the Medication Safety Health-System Manager at Cedars-Sinai Medical Center who oversee the safety and Transitions of Care Program. She completed her pharmacy doctorate and PGY-1 pharmacy practice residency at University of California, San Francisco and then completed the PGY-2 Administrative Residency at Cedars-Sinai Medical Center. In 2017, she was awarded the CSHP Fellowship. At Cedars-Sinai, she has held leadership positions supervising Medical/Surgical nursing units, Regulatory Compliance, and Medication Safety. She was responsible for the safe implementation of the first drug library for the smart infusion pumps, including the PCA and was part of the team that implemented infusion pump and EHR integration. She helps ensures the hospital’s preparation and readiness assessment for TJC/CDPH MERP surveys. She oversees the review of medication adverse events and identifies opportunities for improvement based on trends identified. In the last 5 years, she has expanded the CSMC Transitions of Care program over 100%, created a Transitions of Care 15-hour certificate program with CSHP and initiated a PGY-2 transitions of care residency.
