Learning Objectives:

At the conclusion of this session, participants will be able to:

identify gaps in care and opportunities to improve outcomes for patients with heart failure (Professional, Communicator);
summarize novel data, treatment options, and the clinical implication across the spectrum of heart failure (Scholar, Medical Expert);
assess protocols and initiatives that help to optimize treatments and improve outcomes for patients with heart failure (Scholar, Leader); and
explore strategies to utilize synchronous multi-disciplinary clinical care to improve the management of patients with heart failure (Professional, Health Advocate).

This CHFS symposium will highlight different HF optimization programs and initiatives that exist across the country to inspire delegates to set up similar programs in their own community clinics. The objective is to let cardiologists know of initiatives that exist in other parts of the country that help to improve patient outcomes.

This event is an accredited Group Learning Activity under Section 1 as defined by the Royal College of Physicians & Surgeons of Canada for the Maintenance of Certification program and has been approved by the Canadian Cardiovascular Society. This program was co-developed with the Canadian Heat Failure Society and was planned to achieve scientific integrity, objectivity and balance.

This program is made possible through educational grants from Boehringer Ingelheim and Eli Lilly Alliance and Novartis Canada.
