Learning Objectives:

At the conclusion of this session, participants will be able to:

discuss best practice approaches for multidisciplinary collaboration in the care of patients with pulmonary hypertension to improve clinical outcomes;
advance patient care by ensuring that progression from presentation with dyspnea through testing, diagnosis and treatment is patient-centered and team-based; and
recognize the importance of educating patients, with the support of caregivers and healthcare providers, in improving patients’ self-care skills to improve patient outcomes.

This Session will focus on the diagnosis and management of the patient with pulmonary hypertension with a special emphasis on a multidisciplinary and collaborative care pathway. It will also provide helpful information on how to support patients and their families to improve outcomes.

This event is an accredited Group Learning Activity under Section 1 as defined by the Royal College of Physicians & Surgeons of Canada for the Maintenance of Certification program and has been approved by the Canadian Cardiovascular Society. This program was co-developed with the Canadian Cardiovascular Society and was planned to achieve scientific integrity, objectivity and balance.
